How can a community psychiatrist be useful?
A psychiatric patient often differs from other people only in that he is interested in medical care. Others may equally urgently need it, but for one reason or another avoid it. This often happens, for example, with company executives, school principals, judges and family doctors.
Due to the patterns of thinking handed down from childhood, the administrator can go so far as to lose the ability to delegate authority to subordinates and make decisions. As a result, his subordinates suffer from headaches, back pain, stomach ulcers and mental depression.
The school principal may not be able to tolerate spontaneous behavior and picks up teachers with the same prejudices. Students who may develop school phobias suffer from this. A judge with certain skills learned in childhood may be overly worried about cleanliness and order, and as a result, she will only tolerate orderly and exemplary behavior in herself and others. The family doctor sometimes does not realize the emotional difficulties of his patients and does not always refer them to a psychiatrist when there is evidence for this. Here are some examples of people in powerful positions who can therefore seriously hinder the well-being of other members of the community; the community psychiatrist should be able to influence them.
Community psychiatric education may require people of new professions - perhaps more reminiscent of school teachers than psychiatrists.
An example of this kind of “psychiatric school” is the author’s own experience of teaching an unprepared audience to analyze interactions.
In 1962, ten company managers attended eight weekly hourly lectures on this course. Now this number has risen to thirty-five: from senior administrators to CEOs; they regularly listen to this course, which has turned into a constant type of training for senior staff. Moreover, the lectures were taped and synchronized with the slides illustrating them, so that all 4000 employees of the enterprise got the opportunity to familiarize themselves with the analysis of interactions.
In 1964, a similar lecture series was delivered to some patients in a community psychiatric clinic. Many of them later stated that the lectures “improved” their condition. One of the mothers said: “My daughter is not far behind at school now. I explained some of these things to her and now it’s easier for us. I can help her because I understand myself better.” The worker said that he drinks 50% less and plays cards, that he has made six rationalization proposals (not a single one before) and is getting promoted.
Now, one hundred people listen to these lectures weekly. Many of the students are relatives, friends, and neighbors of former student patients. They proved that many people are able to use their intelligence to improve their lives and work; for this, someone must provide them with an accessible scientific theory about the essence of man.
