It was a joke, if that. In fact, the grass is also a drug. Probably. The fact is that the word itself, in principle, has no universally accepted definition. The last hope of mankind in all controversial health issues, the World Health Organization, generally ignores the issue of the definition of drugs in their Russian-language documents. In English, there is a special dictionary, a list of all the terms that WHO uses, with their interpretation. The word closest to drugs there is drug, but it simultaneously in English means any medicine, so that definition5 got him a long and confusing:
Drug. The term is used differently. In medicine, it refers to any substance that can prevent or cure a disease or improve a physical or mental state. In pharmacology - to any chemical agent that affects the biochemical physiological processes in tissues or organisms. Consequently, it is a substance that is described or can be described in the Pharmacopoeia (official documents controlling the production of medicines).
In everyday use, the term refers to medicinal substances that have a psychoactive effect, and often, even more narrowly, to illegal substances that are not used for medical purposes in addition to any medical. Professional formulations such as “alcohol and other drugs” often imply that the concept is extended to caffeine, tobacco, alcohol and other substances that are often not used for medical purposes, meaning that the intake of these substances is at least in part related to their psychoactive effects.
In general, WHO refuses to take responsibility and washes its hands. So he directly says: “They just don’t call drugs, and there is a logic in everything”. So the question of what to consider drugs, and what is not, in each particular case is solved in accordance with the specific task and personal tastes of the author.
The American Psychiatric Association has developed seven criteria of dependence6 (their documents deal specifically with drugs, but in general you can test your current love with these parameters or, for example, an exciting computer toy):
1) tolerance. Present if an increase in dose is required to achieve intoxication or the desired effect, or if, while maintaining the same dose, the effect of consumption is gradually reduced;
2) withdrawal syndrome. Negative effects upon cessation of consumption, which disappear upon the next intake of this substance or substances close to it;
3) uncontrolled use. The substance is used in large doses or for a longer time than originally planned;
4) there is a clear desire or unsuccessful attempts to stop or at least begin to control the use of the substance;
5) it takes a significant amount of time to find a substance, use it, or recover from the use of this substance;
6) social maladjustment.
Communication, work and rest are affected by the use of the substance;
7) substance intake continues, despite the awareness of the problems associated with it (for example, cocaine-induced depression, aggravated alcohol-related ulcers, and so on).
The worst thing is that it is not necessary that the source of the dependency satisfy all the criteria. Enough to observe at least three. In the case of nicotine, to a greater or lesser extent, all the points except the fifth and sixth are true - smokers are lucky with these two criteria, and this is because the society (for the time being) does not seek to make tobacco use prohibited.
The official “List of narcotic drugs, psychotropic substances and their precursors to be controlled in the Russian Federation” consists of four lists. The first includes substances that are completely prohibited on the territory of Russia, including marijuana. In the second, substances that must be controlled, for example cocaine. The third list is devoted to substances whose turnover can be controlled less carefully - it includes psychotropic drugs, such as pentobarbital. The fourth list includes precursors - substances necessary for the synthesis of drugs, for example, harmless acetic anhydride. Neither tobacco nor alcohol is mentioned in the “List ...”.
One of the most cited publications comparing the harm of different drugs, a review
2007 from the journal Lancet, assesses the damage from each of the twenty substances under consideration according to nine parameters, combined into three categories: harm to health (with occasional use, with constant use, with intravenous administration), dependence (intensity of pleasure from drug administration, degree of psychological and physical dependence) and social damage (the effect of intoxication on behavior, on social life and the total damage to the health care system). Of course, with this approach, it is necessary to take into account not only illegal, but also legal drugs, such as tobacco and alcohol.
According to pharmacologists, doctors, and forensic experts who compiled this review, cigarettes occupy a modest ninth place among drugs (yielding heroin, cocaine, barbiturates, methadone, alcohol, ketamine, benzodiazepines, and amphetamine). In terms of social danger, they are not even among the top ten: a high level of stress on the health care system is balanced by the fact that cigarettes do not cause inappropriate behavior and usually do not prevent a person from working or creating a family. But according to the severity of drug addiction - attention, shock content - cigarettes took the honorable third place, right after heroin and cocaine.
