The last hope of humanity in all controversial medical issues, the World Health Organization, generally ignores the definition of drugs in their Russian-language documents. In English there is a special dictionary, a list of all terms that WHO uses, with their decoding. The drug closest to drugs is the word drug, but it in English simultaneously means any medicine, so the definition5 got to him long and intricate:
Drug. The term is used in many ways. In medicine, it refers to any substance that can prevent or cure the disease or improve the physical or mental state. In pharmacology - to any chemical agent that affects biochemical physiological processes in tissues or organisms. Therefore, this substance, which is described or can be described in the pharmacopeia (official documents controlling the production of drugs).
In everyday use, the term refers to drugs that have a psychoactive effect, and often, even more narrowly, to illegal substances that are not used for medical purposes in addition to any medical substances. Occupational formulations such as "alcohol and other drugs" often imply the extension of this concept to caffeine, tobacco, alcohol and other substances that are often not used for medical purposes, in the sense that the intake of these substances is at least partly related to their psychoactive effects.
In general, WHO refuses to take responsibility and is washing hands. So straight and says: "Drugs that just do not call, and everything has its own logic." So the question of what to consider as drugs and what is not, in each case, is decided in accordance with the specific task and personal tastes of the author.
The American Psychiatric Association has developed seven criteria for dependence6 (they refer specifically to medicines in their documents, but in general you can test your current love or, for example, an entertaining computer toy on these parameters):
1) Tolerance. It is present if, in order to achieve intoxication or the desired effect, an increase in dose is required or if the effect of consumption decreases gradually with the maintenance of the previous dose;
2) withdrawal syndrome. Negative effects when discontinuing use, which disappear when the next intake of this substance or substances close to it;
3) uncontrolled use. The substance is used in large doses or for a longer time than originally planned;
4) there is a clear desire or unsuccessful attempts to stop or at least begin to control the use of the substance;
5) a considerable amount of time is spent on finding a substance, consuming it, or for recovering from the use of this substance;
6) social disadaptation.
Communication, work and leisure are subject to damage because of the use of the substance;
7) substance use continues despite awareness of the problems associated with it (for example, cocaine-induced depression, acute ulcers due to alcohol, etc.).
The most horrible thing is that it is absolutely not necessary that the source of dependence satisfy all the criteria. It is enough that at least three are respected. In the case of nicotine, all points except for the fifth and sixth are more or less valid, with these two criteria smokers were lucky, and even then because society (for the present) does not seek to make tobacco use illegal.
The official "List of narcotic drugs, psychotropic substances and their precursors subject to control in the Russian Federation" consists of four lists. The first includes substances that are completely banned in Russia, including marijuana. In the second - substances whose turnover should be controlled, for example, cocaine. The third list is devoted to substances whose turnover can be controlled less carefully - it includes psychotropic drugs, such as pentobarbital. The fourth list includes precursors - substances necessary for the synthesis of drugs, for example, harmless acetic anhydride. Neither tobacco, nor alcohol in the "List ..." are not mentioned.
One of the most cited publications on comparing the harm of different drugs, a review
2007 from the journal Lancet, assesses the damage from each of the twenty substances under consideration in nine parameters, grouped into three categories: harm to health (with occasional use, with constant use, with intravenous administration), dependence (the intensity of pleasure from the introduction of the drug, the degree of psychological and physical dependence) and social damage (the effect of intoxication on behavior, on social life and total damage to the health system). Of course, with this approach it is necessary to take into account not only prohibited, but also legal drugs, such as tobacco and alcohol.
According to the estimates of pharmacologists, doctors and criminalists who compiled this review, cigarettes occupy a modest ninth place among drugs (yielding heroin, cocaine, barbiturates, methadone, alcohol, ketamine, benzodiazepines and amphetamine) in terms of health damage. On the level of social danger they are not even included in the top ten: the high level of the burden on the health care system is balanced by the fact that cigarettes do not cause inappropriate behavior and usually do not interfere with a person to work or create a family. But the severity of drug dependence - attention, shock-content - cigarettes took an honorable third place, immediately after heroin and cocaine.
