Summing up the data presented in the monograph, the following points should be emphasized. Along with the ever-increasing profiling in medicine, new specialties are being created. And one of the first to isolate obstetric anesthesiology. The first anesthetists in the field of obstetrics and gynecology appeared in the late 50s, and independent departments in obstetric institutions appeared in the mid-60s. There are many reasons for this. First of all, the development and implementation of anesthesia of physiological and complicated births, anesthesia methods for cesarean section and small obstetric operations was required.

Moreover, the main task in this case is “not to harm the fetus and the newborn”, not to cause a violation of the contractile activity of the uterus when using painkillers.

Secondly, the need arose to provide anesthetic care for pregnant women, women in childbirth and puerperas during the treatment of gestosis, bleeding, purulent-septic complications and other pathological conditions in obstetric and gynecological patients.

Experience shows that in obstetrics there are often situations when, despite the high qualifications of obstetrician-gynecologists, it is simply impossible to do without anesthesiologists and resuscitation specialists, especially in critical situations.

The data presented in the work completing a certain stage of research can be considered as the basis for further study in the field of obstetric anesthesiology.

Many questions relating to the problem of DIC, purulent-septic diseases, gestosis, thrombophilic complications and other pathologies that remain to be answered remain unexplained. Without a detailed study of the etiology and pathogenesis of complicated pregnancy, it is difficult to develop preventive and therapeutic measures that can give positive results.

Currently, when the complex therapy of patients with bleeding, purulent-septic diseases and other complications becomes more complicated, it is necessary to conduct treatment together with anesthetists, resuscitators, and transfusiologists. This practice allows you to get the best results in the treatment and prevention of severe diseases.

One of the most important features of obstetric hemorrhage is their unpredictability and suddenness, which requires the provision of emergency and qualified assistance. In this regard, this book proposes new approaches to this problem, methods of "bloodless surgery", a variety of autohemoplasma donations that can prevent complications associated with massive blood transfusion, and the development of multiple organ failure.

Of particular interest are the data on the high efficiency of extracorporeal methods of detoxification, laser and ozone therapy.
These methods provide correction of functional metabolic disorders of homeostasis. Improving the rheological properties of blood leads to a significant improvement and restoration of microcirculation.

In this monograph, the authors do not set the task to consider all issues of anesthesia and intensive care in the field of obstetrics and gynecology, but focused on the most common severe pathological conditions.

Our clinical practice shows that with the development of modern methods of anesthesia and intensive care, it is possible to achieve certain positive results, the joy of motherhood and to exclude possible complications that arise during pregnancy and childbirth.

Knowing the possibilities of endoscopic surgery in gynecology, we can confidently say that the use of modern methods of anesthesia will also reduce the complications caused by the peculiarities of endoscopic correction in gynecological patients.

So, the search for improvement of pain relief in labor, surgical interventions and intensive care is still ahead. The correctly chosen tactics of preventive and therapeutic measures will significantly reduce the risk of complications, reduce their severity, reduce morbidity and mortality caused by various pathologies.

Despite the tendency to increase purulent-septic complications, another pathology, targeted prevention and the use of modern methods of anesthesia and intensive care, if they are carried out in all obstetric institutions and at all stages of the management of pregnant and gynecological patients, will significantly reduce the frequency of complications and save the lives of many women.
