FOREWORD

The medical history chart is intended for third-year students of a medical university. It is necessary for the correct assimilation of the material of the program of propaedeutics of internal diseases, effective work at the patient’s bed and at the end of the course - writing the first student case history

Despite the increasing technicalization and instrumentalization in examining patients under the conditions of modern medicine, the importance of classical diagnostic techniques - detailed questioning and consistent thorough objective research - remains unshakable ("conditio sine qua pop") The very possession of these techniques and their effective application, on the one hand , undoubtedly determines the professional culture of the doctor, and on the other hand, creates that special confidence atmosphere of the doctor’s interaction with the patient, that compliance, on which the ex treatment.

Unfortunately, most of the mistakes that doctors continue to make result from neglect of these classical methods of research, the patient is questioned superficially, examined fluently and often inconsistently

The proposed chart of the history of the disease, developed by a century and a half of the practice of medicine, will help to avoid mistakes, especially at the very beginning of the difficult path of healing

Third-year students quite naturally experience difficulties in correctly recording the medical history they receive at the patient’s bedside data and objective research data, and the first medical histories often turn out to be real “mournful sheets” for the teacher. The presented medical history samples will help to avoid many inaccuracies, first of all , semantically stylistic character

The student’s medical history, unlike the medical one, ends with the sections “Justification of the diagnosis” and “Pathogenesis of Symptoms”, the writing of which allows the student to establish himself in the correctness of his diagnosis, and to the teacher in the depth of the student’s knowledge
