Polyps of the nose

Polyps of the nose is one of the complications of year-round allergic rhinitis. Usually they are localized on the middle nasal concha, around the holes of the maxillary sinuses and ethmoid labyrinth. Polyps are observed in both allergic and infectious rhinitis. Polyps that occur with allergic rhinitis look like whitish or gray, shiny, gelatinous formations. Polyps in chronic infectious rhinitis are hyperemic, granular, dense. Nasal polyps are often observed with cystic fibrosis, so when they are detected, a study of the level of chlorine in sweat is carried out. Nasal polyps are often combined with aspirin intolerance and together with bronchial asthma make up the classic aspirin triad, therefore, in the presence of nasal polyps and asthma, aspirin and other NSAIDs are contraindicated (see
ch. 13, p. VI.B). Polyps of the nose should be differentiated with neoplasms of the nose: papilloma, angiofibroma, squamous cell carcinoma and sarcoma. If these neoplasms are suspected, consultation with an otorhinolaryngologist is indicated. If nasal polyps occur in the presence of allergic rhinitis, the underlying disease is treated, i.e., they eliminate contact with the allergen and prescribe medication (see Ch. 5, p. II.G). If the polyps cause significant inconvenience, prescribe corticosteroids for topical use, for example beclomethasone, flunisolid, mometasone, or corticosteroids for systemic use, for example prednisone, 40-60 mg / day orally for 3-7 days. If the polyps recur, a long course of treatment with topical corticosteroids is prescribed.
