ORGANIZATION AND VOLUME OF OTORINOLARYNOLOGICAL CARE AT THE STAGES OF MEDICAL EVACUATION

The essence of the modern system of medical and evacuation measures for the provision of medical care and treatment of ENT of affected and sick people consists in carrying out sequential and continuous therapeutic measures at various stages of medical evacuation in combination with the evacuation of the wounded and patients to specialized treatment facilities as prescribed.

ENT medical care for the wounded and sick (except for the first and pre-medical) and their treatment is carried out at medical centers and medical facilities, deployed from the front to the rear in a certain sequence. Medical centers and medical institutions received the generalized name of the stages of medical evacuation. Under the stage of medical evacuation we understand the forces and means of medical service (medical centers and medical facilities) deployed on the evacuation routes for receiving, sorting the wounded and sick, providing them with medical care, their treatment and preparation for further evacuation of those in need. In the modern system of treatment and evacuation measures, the timeliness of medical care, the continuity and sequence of treatment measures, that is, the observance of uniform treatment methods for the wounded and sick and the consistent increase in treatment measures at the stages of medical evacuation, are of particular importance. Continuity in the treatment of ENT of the wounded and sick is achieved primarily by a common understanding of the pathological processes that occur in the ENT organs in lesions and diseases, by common methods of their prevention and treatment. A prerequisite for continuity is a clear record of medical records.

In the process of conducting medical and evacuation measures, the troops provide for the provision of the following standard types of medical care: first medical, first-aid (feldsher), first medical and specialized medical care.

Under the guise of medical care, they mean a certain list (complex) of medical and preventive measures carried out in case of defeats and diseases by troops (self-help and mutual assistance) and medical service on the battlefield, in the centers of mass sanitary losses and at the stages of medical evacuation. The type of medical care is determined by the place of delivery, the level of training of the people who provide it, and the availability of appropriate equipment. The volume of medical care is the totality of medical and preventive measures within the boundaries of a specific type of medical care performed at the stages of medical evacuation in relation to certain categories of wounded and sick for medical reasons and in accordance with the military and medical situation.

Medical sorting of the wounded and sick (including the ENT of the affected) is the most important event, providing a clear organization of the stages of medical evacuation to provide medical care to the wounded and sick.

Medical sorting is the distribution of the wounded and sick into groups according to the need for uniform medical-evacuation and preventive measures. The content of these measures is established in accordance with medical indications and the amount of medical care that can be provided at this stage of medical evacuation in specific conditions. As a result of sorting at the stages of medical evacuation, the following main groups of ENT of the wounded should be distinguished:

- representing a danger to others (infected with toxic substances, bacterial agents, radioactive substances with radiation dose rates exceeding safe doses, in a state of sharp psychomotor agitation), and therefore subject to special treatment or isolation;

- in need of medical care at this stage of medical evacuation;

- subject to further evacuation without medical assistance;

- who have received defeats that are not compatible with life, and need only care (agonizing);

- subject to return to their units (after receiving medical care and a short rest).

There are two types of medical sorting: intra-point and evacuation transport. Intra-point sorting is intended to distribute the incoming wounded into groups for referral to the appropriate functional units of this stage of medical evacuation and establish the sequence of their referral to these units. The evacuation and transport sorting is carried out with the aim of distributing the wounded into groups in accordance with the direction, priority, ways and means of their further evacuation.
